Job summary

Keystone Ridge Post Acute Nursing and Rehabilitation in Omaha, NE is seeking a physical therapist to join our in-house rehab program as a full-time team member. You will screen and evaluate patients, develop care plans, and deliver rehabilitative interventions to improve mobility and function in a geriatric population.

This role involves standing and walking throughout the facility, with lifting and repositioning as needed.

Qualifications

• Physical Therapist license required.
• Open to PTs at all experience levels; new grads welcome.

Responsibilities

• Screen and evaluate patients with physical functioning disorders and/or wound care; develop care plans.
• Deliver rehabilitative, skilled, and medically necessary treatment interventions.
• Perform standing/walking tasks throughout the facility with occasional lifting and repositioning.
• Administer procedures as part of rehabilitation including manual techniques, ambulation, therapeutic exercises, modalities, devices, and wound care.
• Provide consultation and counseling to patients, families, caregivers, and service providers.
• Generate comprehensive discharge summaries ensuring regulatory compliance.
