Job summary

Angels Care Home Health is seeking a dedicated Physical Therapist to assess, plan, and implement individualized therapy programs. You will restore or maintain patients’ well-being by applying evidence-based PT techniques and coordinating with an interdisciplinary care team across multiple locations.

Applicants should hold an active Physical Therapist license, have at least two years of PT experience, and be CPR certified with Hepatitis profile.

Qualifications

• Current state license as a Physical Therapist.
• Two years of experience as a Physical Therapist.
• Current CPR and Hepatitis profile.
• Knowledge of durable medical equipment.

Responsibilities

• Plans, implements, and evaluates patient care plans to restore or maintain patient well-being.
• Plans, organizes, and conducts physical therapy treatment programs based on the medical referral and their evaluation.
• Collaborates with interdisciplinary team.
