Job summary

Post Rehabilitation Services' Agawam Campus is seeking a motivated Physical Therapist (PT) to join our rehabilitation team in a skilled nursing facility. You will evaluate residents, develop individualized treatment plans, and focus on gait training, balance, strength, and mobility to improve independence.

This full-time position offers competitive hourly pay of $45-$50, collaboration with PT/OT/SLP and nursing staff, and opportunities for professional development and advancement.

Qualifications

Active Physical Therapist license required.
Strong communication and clinical reasoning skills.
Skilled nursing experience preferred but not required.

Responsibilities

Complete physical therapy evaluations and establish individualized treatment plans.
Provide therapy focused on gait training, transfers, balance, strengthening, endurance, and pain management.
Collaborate with PTA staff, nursing, physicians, and interdisciplinary team members to support resident goals.
Monitor resident progress and adjust treatment plans as clinically appropriate.
Maintain timely and accurate documentation in accordance with facility standards.
