Job summary

Continuum Therapy Partners is seeking a Physical Therapist in Midlothian, VA. The role requires a Doctor of Physical Therapy (DPT) and an active PT license, with NPTE successfully completed.

Prior PT experience is preferred, and you will work to create and adjust individualized treatment plans in collaboration with a multidisciplinary team. Responsibilities include documenting progress, educating patients and families on home exercise programs, and contributing to outcomes-based care.

Qualifications

• DPT degree required.
• Current, unencumbered state Physical Therapy License; must pass the NPTE.
• 1 year of previous experience as a Physical Therapist preferred.

Responsibilities

• Evaluates, develops, and implements individualized treatment plans in coordination with other disciplines.
• Documents patient progress and adjusts plans of care based on clinical outcomes.
• Educates patients and families on home exercise programs and injury prevention.
