Job summary

Union Post Acute in Union, SC is hiring a Full-time Physical Therapist Assistant to join our skilled nursing and rehabilitation team. You will help residents regain independence, improve mobility, and return to activities they love, within a warm, home-like environment.

As part of a supportive team, you’ll implement individualized treatment plans, provide therapeutic exercises, monitor progress, and educate residents and caregivers.

Qualifications

• Active PTA license in good standing in South Carolina.
• Graduate of an accredited Physical Therapist Assistant program.
• Compassionate, patient-focused professional with strong communication.
• Excellent teamwork and organizational abilities.
• Ability to work with older adults recovering from illness or surgery.

Responsibilities

• Assist with implementing individualized treatment plans developed by the Physical Therapist.
• Provide therapeutic exercises and functional mobility training to improve strength, balance, coordination, and endurance.
• Help residents improve walking, transfers, and daily activities.
• Monitor responses to treatment and communicate progress to supervising PT.
• Document treatments and resident progress accurately and timely.
• Educate residents and caregivers on safe mobility techniques and activities.
• Collaborate with nursing and rehabilitation teams to support positive outcomes.
• Provide compassionate, respectful care while maintaining residents' dignity and safety.
