Job summary

Avamere Living in Medford, Oregon is seeking a Full-Time Physical Therapist Assistant to join our rehab team under licensed therapists. You will assist in providing quality patient care and implement established treatment plans to support patient progress.

Schedule is Monday through Friday, 8 to 5 at Avamere at Three Fountains, 835 Crater Lake Avenue. You will collaborate with nurses, physicians, and support staff to achieve optimal outcomes and maintain documentation as required.

Qualifications

  • Graduate of an approved Physical Therapy Assistant curriculum.
  • Active CPR/BLS during employment.
  • In good standing with regulatory agencies/licensing boards.
  • Working knowledge of Medicare and other payer sources.
  • Able to practice unencumbered.
  • Exudes professionalism in presentation.
  • Ability to multitask.
  • Must maintain licensure as required.

Responsibilities

  • Implements and executes treatment plans under supervision of a licensed Physical Therapist.
  • Communicate with patients, families, referring physicians, and team members.
  • Provides timely written documentation per facility and department requirements.
  • Develops and enhances clinical and professional skills.
  • Provides documentation for insurance, licensing, regulatory, and reimbursement requirements.
  • Supports and participates in department operations and development.
  • Reports changes in patient status to supervising therapist promptly.
  • Prepares and submits patient charges for billing.
  • Meets productivity standards as established by the Supervisor.
  • Ensures safety of self, patients, and others.
  • Attends required meetings and training sessions.
  • Coordinates treatment goals with team members.
  • Maintains licensure and awareness of patient/community needs.

Job Summary

The Primary purpose of the Physical Therapist Assistant is to assist in providing appropriate aspects of quality patient care under the supervision of licensed therapists.

Key Responsibilities
  • Implements and executes established patient treatment plans under the supervision of a licensed Physical Therapist.
  • Communicate with patients, families, referring physicians, and other members of the team.
  • Provides timely written documentation per facility and department requirements.
  • Develops and enhances clinical and professional skills through knowledge and professional associations.
  • Provides timely written documentation as required by insurance provider to meet licensing, certification, regulatory, and reimbursement requirements.
  • Supports and participates in department operations and development.
  • Reports any changes in patient status or needs to supervising therapist on a timely basis.
Associate Responsibilities
  • Prepares and submits patient charges accurately to provide appropriate billing information.
  • Meets or exceeds the productivity standards as established by the Supervisor.
  • Uses professional judgment to ensure safety of self, patients, and others at all times.
  • Achieves quality patient care through interdisciplinary communication.
  • Attends required meetings as designed by the Director of Rehab and/or Supervisor.
  • Effectively communicates with therapists concerning patients' response to treatment plan.
  • Provides constructive feedback to rehab aides.
  • Attends in-services, training sessions or other educational presentations.
  • Coordinates treatment goals with other team members.
  • Maintains valid licensure, certification, or equivalent as requested by appropriate state agency.
  • Maintains awareness of patient and community needs, conditions, and programs.
