Job summary

Northwell Health is seeking a licensed Physical Therapy Assistant to assist in administering therapy care programs and monitor patient progress. You will maintain medical records and communicate with the Physical Therapist and other health personnel.

Responsibilities include direct patient care, documentation, and participation in quality improvement activities. NY licensure or limited permit is required, with 1–3 years of relevant experience.

Qualifications

• Associate's Degree required or equivalent experience.
• Current NY PTA license required; or current limited permit for outpatient settings.
• Specialized certifications, as needed.
• 1-3 years of relevant experience required.

Job Responsibility

1. 1.Administers direct patient care in accordance with the written and signed prescription of the referring physician or other State authorized practitioner, as directed.
2. 2.Submits required documentation in accordance with the policies and procedures of the department.
3. 3.Maintains progress notes and discharge summaries of assigned patients.
4. 4.Responds appropriately to established department emergency procedures for patient safety and care when necessary.
5. 5.Participates in quality improvement chart reviews and activities.
6. 6.May attend family meetings/conferences pertaining to the welfare and management of assigned patients.

Job Qualification

1. 1.Associate's Degree required, or equivalent combination of education and related experience.
2. 2.Current License to practice as a Physical Therapy Assistant in New York State, required.

   Current limited permit to practice as a Physical Therapy Assistant in New York State, required (applicable to outpatient settings only).
3. Specialized certifications, as needed.
4. 3.1-3 years of relevant experience, required.
