Salary: $100,000 - $140,000 + $8,000 Sign-On Bonus!

Qualifications:

To excel in this role, you'll need:

• A current PT / Physical Therapy license in the State of Minnesota.
  
• Experience in a home care setting, including proficiency in OASIS documentation.
  
• A valid Driver's License and reliable transportation.
  
• Exceptional communication and interpersonal skills.
  
• The ability to work autonomously as well as collaboratively within a team.
  
• Proficiency in Microsoft Office programs.
  
• Availability to travel to patient homes in the local area.
