Coronado Healthcare in Phoenix, AZ is seeking a full-time Physical Therapist to join our in-house rehab program. Compensation includes a $5,000 sign-on bonus and pay ranging from $45 to $54 per hour based on experience. New grads welcome to apply.

Qualifications

Responsibilities

• Screen/evaluate patients with cognitive and/or ADL physical functioning disorders and develop a plan of care following regulatory and clinical practice standards.
• Use standardized assessments and evidence-based practice to support clinical interventions.
• Provide rehabilitative, skilled, and medically necessary treatment per regulatory and clinical practice requirements.
• Administer procedures as part of the rehab plan including ADL retraining; therapeutic activities; exercises; modalities; devices.
• Assess progress and adjust treatments in the clinical record per regulatory and clinical practice requirements.
• Evaluate treatment outcomes; modify services and make referrals as needed.
• Provide consultation to patients and families and other service providers.
• Communicate with supervisor and health team regarding progress and plans.
• Provide discharge summaries per regulatory requirements.
