Full Time Physical Therapist - $5k sign on bonus

Skilled Nursing Facility: A leading provider of rehabilitation services for the elderly with many locations throughout the country. Our dedicated professionals are committed to enhancing our patients' quality of life with passion and integrity.

The Staff Physical Therapist evaluates and treats patients/residents, communicates with families, physicians, and other health team members, and maintains documentation of services in the medical records. Provides Physical Therapy services including treatment planning & implementation, and discharge planning. Tests patients’ physical abilities, analyzes rehabilitation goals, and consults with the rehab team to coordinate the most effective physical therapy program.

Organizational Relationships: Supervises: Responsible for the care delivered by PTAs and Rehab Technicians.

Responsibilities include:

1. Evaluating patients/residents within 24 hours of physician referral.
2. Developing effective treatment plans and obtaining approval from the referring physician, then treating patients accordingly.
3. Communicating regularly with supervisors, the rehabilitation team, and nursing staff regarding treatment plans, progress, and prognosis.
4. Recording evaluations, treatment notes, progress reports, and discharge summaries as per policy.
5. Instructing patient/resident families or nursing staff in maintenance programs.
6. Securing necessary durable medical equipment to facilitate independence in mobility.
7. Submitting logs and billing information timely.
8. Reporting equipment or problem issues immediately.
9. Complying with licensing and regulatory guidelines.
10. Performing other duties as assigned.
