A leading provider of rehabilitation services seeks a Staff Physical Therapist who will evaluate and treat patients, develop treatment plans, and collaborate with health team members. This role involves maintaining thorough documentation and ensuring compliance with state guidelines while delivering quality care in skilled nursing facilities.

The Staff Physical Therapist evaluates and treats patients/residents, communicates with families, physicians, and other health team members, and maintains documentation of services in the medical records. Provides Physical Therapy services including treatment planning & implementation, and discharge planning. Tests patients' physical abilities, analyzes rehabilitation goals; consults with the rehab team to coordinate the most effective physical therapy program.
Organizational Relationships: Supervises: Responsible for the care delivered by PTAs and Rehab Technicians.

Licensure/Certification: Bachelor's degree (or foreign equivalent) in Physical Therapy and a current license or recent completion of exams.
