Job Description

The Staff Physical Therapist evaluates and treats patients/residents, communicates with families, physicians, and other health team members, and maintains documentation of services in the medical records. Provides Physical Therapy services including treatment planning & implementation, and discharge planning. Tests patients' physical abilities, analyzes rehabilitation goals; consults with the rehab team to coordinate the most effective physical therapy program.

Organizational Relationships: Supervises: Responsible for the care delivered by PTAs and Rehab Technicians.

  • Evaluate patients/residents within 24 hours of physician referral.
  • Develop effective treatment plans and obtain approval from the referring physician, then treat patients/residents accordingly.
  • Communicate regularly with supervisors, the rehabilitation team, and nursing staff regarding treatment plans, progress, and prognosis.
  • Record evaluations, daily treatment notes, weekly progress notes, 14-day progress reports to physicians, and discharge summaries per policy and procedure.
  • Instruct patients'/residents' families or nursing staff in maintenance programs.
  • Secure necessary durable medical equipment to facilitate independence in mobility, such as orthoses, walkers, canes, elevated toilet seats, and grab bars.
  • Submit logs and billing information promptly.
  • Report any equipment or problem areas within each facility to the supervisor immediately.
  • Comply with the State Licensing Board of Arizona, Title 22, CMS, and Arizona Labor Board Guidelines.
  • Perform other duties as assigned.
