Job summary

Uplands Village in Pleasant Hill, TN is seeking a Full-time Physical Therapist to screen, evaluate, and revise plans of care under physician supervision, delivering high-quality therapy to residents in a peaceful senior living setting.

The role requires a PT degree (B.S./M.S./D.P.T.) and an active license, with strong organizational and communication skills and the ability to supervise Physical Therapy Assistants. A $7,500 sign-on bonus is offered for direct applicants.

Qualifications

• Bachelor’s, Master’s or Doctorate degree in physical therapy from an accredited program.
• Active state license required.
• Excellent organizational and time management skills.
• Excellent written and verbal communication skills.
• Ability to supervise Physical Therapy Assistants.

Responsibilities

• Screen, evaluate, re-evaluate, discharge plan and revisions in plan of care as supervised by physician.
• Provide therapy services to residents and implement established plans of care.
• Adhere to Functional Pathways policies and procedures and complete required screenings and licensure.
