Job summary

Enhance Rehabilitation is currently seeking qualified Physical Therapists to join our dynamic team in a SNF setting. As an Enhance Rehabilitation Physical Therapist, you will evaluate and treat patients following an individualized plan of care.

You will collaborate with facility staff to ensure excellent care for your patients, supervise Physical Therapist Assistants, and re-evaluate progress while documenting evaluations, re-evaluations, discharges and daily progress notes to support

• Collaborate with facility staff to ensure excellent care for you patients
• Evaluating new patients, developing plans of care and setting measurable, functional goals
• Treating patients, answering their concerns and educating them on their injury, progress and probable outcomes
• Supervising Physical Therapist Assistants
• Re-evaluating patient progress, revising plans of care and resetting goals
• Complete timely and accurate documentation including; evaluations, re-evaluations discharges and daily progress notes.
