Job summary

Heritage Community of Kalamazoo in Kalamazoo, MI is seeking a Full-time Physical Therapist to join our therapy team. You will screen, evaluate, revise plans of care, and discharge plans under physician supervision.

We offer a $10,000 sign-on bonus for direct applicants, a Monday–Friday schedule, and a supportive environment with opportunities to supervise PTAs and advance your clinical skills.

Qualifications

• Bachelor's, Master's or Doctorate in physical therapy from an accredited program.
• Active state license and required certifications per state/national boards.
• Excellent organizational and time management skills.
• Excellent written and verbal communication skills.
• Ability to supervise Physical Therapy Assistants.
• Completion of screenings, certifications and licensure; varies by location.

Responsibilities

• Screen, evaluate, and re-evaluate patients and discharge plans.
• Revise established plans of care under physician supervision.
• Collaborate with the therapy team to support resident care.

Job Summary:

The Physical Therapist shall screen, evaluate, re-evaluate, discharge plan, and make revisions in any established plan of care as supervised by the physician.

Qualifications:

• Bachelor's, Master's or Doctorate degree in physical therapy from an accredited program.
• Certification and licensure requirements as established by state and national board associations.
• Active state license required.
• Excellent organizational and time management skills
• Excellent written and verbal communication skills
• Possess current working knowledge of Functional Pathways policies and procedures, clinical programs with application skills to therapy disciplines.
• Ability to supervise Physical Therapy Assistants
• Completion of all required screenings, certifications, and licensure. Screening requirements may vary by location.
