Position: Full-Time Licensed Physical Therapist (PT)

Join our growing home health team serving the Indiana/Blairsville area. The VNA has provided skilled nursing and therapy services since 1970.

Benefits and wages may vary by location. Minimum hours per week or status may be required for certain benefits.

Your schedule: Typical hours are Monday-Friday from 8am-5pm, with flexibility for other arrangements. Some weekends, evenings, and holidays may be required.

Qualifications:

• Graduate of an accredited school of physical therapy (American Physical Therapy Association accredited)
• Minimum 6 months experience in acute care or rehabilitation; outpatient experience considered if involving medical/surgical patients
• Licensed as a Physical Therapist in Pennsylvania
• Reliable transportation, valid driver’s license, and adequate insurance

Responsibilities:

• Perform initial assessments within 24 hours of referral
• Conduct comprehensive evaluations of patients' muscle, nerve, joint, and functional abilities
• Develop care plans collaboratively with physicians, patients, and team members, updating as needed
• Instruct patients and families on therapy programs and the use of assistive devices
• Supervise home health aides biweekly when designated as the coordinating health professional
• Manage patient admissions and care coordination upon physician request
• Plan discharge procedures with patients and families
• Complete all required assessments and documentation, including OASIS, and monitor vital signs
