Job summary

Northwell in New York seeks a Physician Assistant or Nurse Practitioner to perform comprehensive patient evaluations, develop and communicate care plans, and participate in rounds and discharge planning. Qualifications

• Bachelor's degree required; ARC-PA accredited program graduate.
• Current NY license to practice as PA and NCCPA certification required.
• Current BLS certification required.
• Master's Degree in Nurse Practitioner program and NBCC certification in specialty area required.
• Current NY NP license and NY RN license required.
• RNFA required for acute care NP; ACLS.

Responsibilities

• Take initial patient histories and perform exams.
• Document and communicate plan of care; follow up to ensure screening/prep.
• Perform daily patient rounds.
• Write orders for medications, labs, and tests.
• Interpret laboratory/test results.
• Consult with attending physicians and care team to ensure quality care.
• Upon discharge, note summaries, prescriptions, referrals on charts.
• Inform patients of post-discharge care instructions.
