Job summary

Avir Vista in Crockett, TX is seeking a Full-Time Occupational Therapist to join our in-house rehab team. You will assess residents, develop treatment plans, and deliver evidence-based therapy to improve function and independence.

You will collaborate with the interdisciplinary team, document accurately, and ensure compliance with state and facility policies. Active Texas OT license required; SNF/long-term care experience preferred; new grads welcome.

Qualifications

• Active Texas Physical Therapy Assistant license
• Good standing with all licensing boards and regulatory agencies
• SNF or long-term care experience preferred
• Familiarity with Net Health documentation a plus
• Strong interpersonal and communication skills

Responsibilities

• Evaluate residents to determine functional impairments and therapy needs.
• Develop and implement individualized treatment plans focused on ADLs and IADLs.
• Track progress and modify treatment plans as needed in coordination with the IDT.
• Recommend adaptive equipment and home modifications to promote independence.
• Educate patients, caregivers, and families on therapeutic strategies.
• Ensure accurate, timely documentation per state, federal, and facility guidelines.
• Participate in team meetings and care planning activities.
• Adhere to all regulatory and facility policies.
