Tender Touch Rehab Services in Naugatuck, CT is seeking a licensed Occupational Therapist for a full-time position in a skilled nursing/long-term care setting. Compensation is $42.00–$48.00 per hour and requires NBCOT certification with current state licensure.

You will evaluate and treat patients with functional, cognitive, and ADL impairments; create personalized care plans; collaborate with the care team; and maintain documentation to meet regulatory requirements while ensuring physician

Qualifications

• Degree in Occupational Therapy (BS, MS, MA, or OTD) from an AOTA-accredited program.
• Certified by NBCOT as an OTR, if required by the state.
• Current license as an Occupational Therapist in the state of practice.

Responsibilities

• Evaluate and treat patients with functional, cognitive, and ADL impairments.
• Create and implement personalized care plans.
• Collaborate with team members to ensure quality outcomes.
• Maintain documentation to meet all regulatory requirements.
• Ensure all required physician orders are obtained and up to date.
