Monte Vista Hills Healthcare in Pocatello, ID is seeking a full-time Occupational Therapist to screen and treat patients with cognitive and ADL impairments. You will develop care plans, use standardized assessments, and provide therapy and discharge summaries in accordance with regulatory standards.

Qualifications

• Occupational Therapist license is required.
• Open to Occupational Therapists at all experience levels. New Grads are welcome!

Responsibilities

• Screens/evaluate patients with cognitive and/or ADL disorders and develops plan of care.
• Uses standardized assessments and evidence-based practice to support interventions.
• Provides rehabilitative and medically necessary treatment per requirements.
• Administers procedures as part of rehab plan (ADL retraining, therapy, modalities, etc).
• Assesses progress and adjusts treatments in clinical record.
• Evaluates outcomes and modifies services based on progress and referrals.
• Provides counseling to patients, families, and other providers.
• Communicates with supervisor and health team regarding progress and plans.
• Provides comprehensive discharge summaries per regulatory requirements.
