Qualifications

• Licensed OT with eligibility to practice in Texas.
• Open to applicants of all experience levels.

Responsibilities

• Screen and evaluate patients with cognitive and ADL functioning disorders and develop plans of care.
• Use standardized assessments and evidence-based practice to support interventions.
• Provide rehabilitative and medically necessary treatment for cognitive/ADL disorders per regulatory standards.
• Administer procedures as part of the rehabilitation plan (ADL retraining, exercises, modalities, assistive devices).
• Monitor patient progress and adjust treatment plans in the medical record.
• Evaluate outcomes and refer as needed based on results.
• Offer counseling to patients and families about the treatment process.
• Communicate progress, barriers, and plans with the supervising clinician and health team.
• Prepare a comprehensive discharge summary in line with clinical requirements.
