Job summary

Concept Rehab, Inc. in Battle Creek, MI is seeking a dedicated Occupational Therapist (OT) to join our rehabilitation team. This full-time role offers collaboration with a multidisciplinary care team to help patients achieve greater independence and quality of life.

You will evaluate patients, develop individualized OT treatment plans, provide skilled OT services, educate patients and families, and document progress. Previous rehab or skilled nursing experience is a plus.

Qualifications

• Graduate of an accredited Occupational Therapy program.
• Current Michigan OT license or eligibility for licensure.
• Strong clinical and communication skills.
• Compassionate, dependable, and team-oriented attitude.
• Previous rehabilitation or skilled nursing experience is a plus.

Responsibilities

• Evaluate patients and develop individualized OT treatment plans.
• Provide skilled OT services based on patient needs and goals.
• Implement therapeutic activities to improve function, mobility, and independence.
• Monitor patient progress and modify treatment plans as appropriate.
• Complete timely and accurate documentation.
• Collaborate with PT, SLP, nursing, physicians, and other care team members.
• Educate patients, families, and caregivers as appropriate.
