Job summary

Champion Rehab is seeking an Occupational Therapist to join our growing team in Minnesota. The role offers full-time hours with opportunities for growth across inpatient, outpatient, and home health settings, plus career advancement across multiple care environments.

You will provide direct patient care, collaborate with an interdisciplinary team, educate families and staff, and ensure accurate documentation and discharge planning in line with regulatory guidelines.

Qualifications

• Must be graduate of an accredited OT program.
• Currently licensed to practice in State of MN.
• New graduates and temporary license holders are encouraged to apply.
• Ongoing attendance of educational seminars to maintain license(s) as required by practice acts for CEU credits.
• Must maintain work and client/resident confidentiality.

Responsibilities

• Direct patient care: assessment of patient needs, treatment, and ongoing plan modifications.
• Collaborate with interdisciplinary team to maximize patient satisfaction and outcomes.
• Provide clinical guidance including family/staff education and discharge planning; supervise OTA; communicate with Physicians for care fluency.
• Complete documentation following state and federal regulatory guidelines.

As an Occupational Therapist you'll:

• Direct patient care: continuous assessment of patient needs and functional deficits, patient care/treatment, on-going modifications to existing plan of care as needed, ability to initiate appropriate referrals to other disciplines when needed
• Work closely with skilled and dedicated interdisciplinary team in order to maximize patient satisfaction and overall success based on diagnosis and comorbidities
• Apply clinical expertise as necessary including but not limited to family and staff education/training, recommendations and guidance for safe and successful discharge, supervision of Certified Occupational Therapy Assistant, communicating with Physicians to ensure fluency of care
• Complete documentation which follows regulatory guidelines (state, federal)
