MEDICAL PROVIDERS PARTNERS PLLC is seeking an organized, motivated Medical assistant/Chronic Care Coordinator to support patients in our medical practice. The Chronic Care Coordinator will work closely with patients, physicians, nurse practitioners, office staff, and other healthcare professionals to ensure patients with chronic medical conditions receive appropriate ongoing care and follow-up.

• Experience in healthcare, medical-office operations, patient coordination or a related field.
• A valid driver’s license, reliable transportation, and ability to travel to assigned locations.
• Excellent communication and interpersonal skills.
• Strong organizational and time-management skills.
• Strong computer skills and ability to work with an EMR.
• Ability to manage multiple patients and tasks simultaneously.
• Strong attention to detail and documentation.
• Ability to work independently and as part of a healthcare team.
• Understanding of patient confidentiality and HIPAA requirements.

Responsibilities

• Conduct monthly outreach to patients enrolled in the Chronic Care Management program.
• Assist with patient navigation tasks including prior authorisation resolution, referral routing and scheduling, medication refill request, DME supply ordering and transportation arrangements.
• Provide patient education on current medical conditions and medication.

Job Summary

We are seeking an organized, motivated, and dependable Medical assistant/Chronic Care Coordinator to support the patients in our medical practice.

The Chronic Care Coordinator will work closely with patients, physicians, nurse practitioners, office staff, and other healthcare professionals to ensure patients with chronic medical conditions receive appropriate ongoing care and follow-up.
