La Jolla Post-Acute in San Diego is seeking a compassionate Licensed Vocational Nurse (LVN) to join our skilled nursing team. You will provide direct nursing care according to care plans and physician orders, administer medications, and support residents' health and safety.

This role collaborates with RNs, physicians, CNAs, and other professionals to maintain a resident-centered environment, ensure accuracy in documentation, and uphold infection prevention and safety standards.

Formación

• Active California LVN license.
• Experience in a skilled nursing facility preferred.
• Knowledge of medication administration and nursing procedures.
• Strong communication and documentation skills.

Responsabilidades

• Provide direct nursing care to residents per care plans and orders.
• Administer medications and treatments as prescribed.
• Monitor residents' conditions and report changes promptly.
• Document observations and care provided accurately.
• Monitor vital signs and coordinate with the team.
• Supervise and support CNAs within policy.
• Follow infection prevention, safety, and documentation standards.
• Communicate professionally with residents, families, and team.
