Qualifications

• California LVN license required
• High School Diploma or equivalent
• BLS or CPR certification required
• General computer skills incl. Word/Excel/Outlook and EMR data entry

Responsibilities

• Serve as primary contact and coordinator of care for assigned patients
• Assist patients with social service needs
• Present patients to provider teams during care reviews
• Administer medications and injections; check patient’s blood glucose
• Document encounters in the health record system
• Maintain clinic supplies and unitized kits
