Job summary

The LPN will provide compassionate nursing care, follow care plans, administer medications, and document accurately in a fast-paced sub-acute and long-term care setting. Requirements include an active LPN license in Virginia and a valid BLS certification.

Qualifications

• Active LPN license in the state of VA.
• BLS Certification (CPR) required.
• Ability to care for and manage multiple patients in a fast-paced healthcare environment.
• Strong clinical judgment and attention to detail.

Responsibilities

• Provide patient care per accepted nursing standards and plan of care.
• Follow individualized care plans developed by Nurse Manager and team.
• Perform medication administration and treatments per plan and policies.
• Monitor residents’ conditions and report changes promptly to supervising staff.
• Maintain accurate documentation per physician orders and care plans.
