Responsibilities

• Prepares, monitors, and administers medication to Residents at scheduled intervals.
• Assist Residents diagnosed with diabetes at regular intervals.
• Review notes in the Resident Service Plans Binder, Resident Communication Logs with staff.
• Review Medication Administration Records for accuracy.
• Complete Comprehensive Assessments and related screenings for all Residents.
• Notify Pharmacy upon Resident move-out or hospital admission to stop or adjust meds.
• Coordinate care plans for Residents returning from hospital.
• Perform regular audits of medications and assist with ordering.
