Job summary

WPM is seeking a Wellness Nurse LPN to promote health and well-being in an assisted living community. You will perform nursing assessments, manage medications, monitor residents, and coordinate care with a multidisciplinary team.

Qualifications

• Current Licensed Practical Nurse (LPN) license in good standing.

Responsibilities

Position: Wellness Nurse LPN
Pay: $34.00 per hour

Position Summary

The Wellness Nurse is responsible for promoting the health, safety, and well-being of residents in an assisted living community. This position provides nursing assessments, medication management, health monitoring, and coordination of care while ensuring compliance with state regulations and community policies. The Wellness Nurse works closely with residents, families, physicians, caregivers, and the leadership team to deliver compassionate, resident-centered care.
