Position Summary

This per diem position offers an opportunity for the LVN to work under the direction of a Registered Nurse, providing skilled nursing and palliative care to patients at their residence. The LVN works cooperatively with other Interdisciplinary Team members, providing intermittent visits or shifts of skilled nursing care, including pain management/symptom control and nursing observations. The LVN provides information about the expected course of illness, care techniques, and other health measures to patients, family members, or primary caregivers. The LVN participates in care plan development and implementation. Must be able to utilize computers and be comfortable with electronic medical records.

Qualifications
  • Graduate of an accredited school of vocational nursing.
  • Current LVN license within the State of California. Certification in Hospice and Palliative Nursing encouraged.
  • At least one year of LVN experience.
  • Valid driver's license, automobile insurance, reliable and well-maintained transportation.
  • Spanish speaking is a plus but not required.
