We are hiring for a Full-Time Licensed Practical Nurse (LPN) and a PRN Weekend On-call LPN (typically every 3-4 weekends) at Jeff Davis MD Home Care, part of LHC Group. Responsibilities

The Licensed Practical Nurse (LPN) in Home Health provides quality patient care and performs skilled technical tasks in the patient's home environment.

• Provides services within the scope of practice as defined by state laws, following the plan of care, and coordinating with the healthcare team. May not alter the plan of care.
• Instructs patients' families in care and maintaining a healthy environment.
• Observes, records, and reports the patient's response to treatment and any changes to the supervising nurse or physician daily.

Education and Experience

License Requirements

• Current LPN license in the state of practice.
• Valid driver's license, vehicle insurance, and access to reliable transportation.
• CPR certification required.

Additional State-Specific Requirements

• Current unrestricted LPN licensure in LA.
• Minimum of one year of experience as an LPN.
