Job summary

Live Well Homecare seeks an experienced Infection Control Nurse to support high-quality patient care through infection prevention programs across our facilities. This role emphasizes surveillance, policy development, and regulatory compliance.

The nurse will review medical records, coordinate rounds, and participate in outbreak investigations, collaborating with public health agencies to maintain NHSN reporting standards.

Qualifications

• Two years of hospital experience required.
• BSN degree required; RN license required.
• Experience in infection prevention, survey readiness, and epidemiology preferred.
• Strong understanding of surveillance, infection control principles, and regulatory requirements.

Responsibilities

• Support infection prevention and control activities across facilities.
• Develop, review, and revise infection control policies and procedures.
• Participate in surveillance to evaluate infection trends and program impact.
• Ensure compliance with Joint Commission, DOH, and CDC requirements.
• Collaborate with external agencies and assist in outbreak investigations.
• Review medical records for infection-related data and reporting.

Assists in developing and implementing Infection Control Programs, coordinating rounds, and recommending improvements to infection prevention practices. Serves as a resource for infection control initiatives, collaborates with community health agencies, and participates in investigations of reportable infections. Maintains expertise in CDC NHSN reporting criteria and conducts medical record reviews to identify hospital-acquired infections. Participates in continuing education and health-related associations to stay current in the field.
