An established industry player in home healthcare is seeking compassionate and experienced Registered Nurses and LPNs to join their in-home care team. Job Overview:
We are looking for a compassionate and experienced Registered Nurse (RN) or Licensed Practical Nurse (LPN) to join our in-home care team, supporting clients living with Alzheimer’s disease and other forms of dementia. This role is ideal for a nurse who is patient, attentive, and understands the unique needs of individuals with memory loss. This position works closely with the Nurse Care Manager to provide support for patient and family.
Key Responsibilities:
Deliver one-on-one nursing care in a home environment
Provide medication administration and health monitoring
Support clients with activities of daily living and personal care
Use memory care best practices to support cognitive function and emotional well-being
Monitor for changes in condition and communicate with care team
Provide family education and emotional support
Maintain accurate and timely documentation
Current Illinois Nursing License
4 years or more of Nursing Experience
Must possess the ability to navigate a phone application to clock in and out, as well as enter case notes
Demonstrate the ability to be a strong team player
Ability to work independently, and when circumstances warrant, the ability to make decisions
Previous home health experience preferred but not required
Provide skilled nursing care to patients in a home environment
Manage and monitor feeding tubes and nutritional plans
Administer medications and treatments per care plan
Assist with daily living activities, mobility, and hygiene
Monitor for changes in condition and report to the care team
