Job summary

LiveWell Homecare Agency seeks an experienced hospice RN to evaluate patients, develop and implement personalized care plans, and coordinate with an interprofessional team to promote comfort and dignity for patients and families.

You will provide specialized nursing procedures, education, and advocacy, supporting a multidisciplinary team including arts therapies, while adhering to safety protocols and maintaining high-quality care standards.

Qualifications

  • Associate’s Degree in Nursing (RN license required).
  • Minimum of 2 years of experience; experience in hospice preferred.
  • Valid driver’s license and ability to travel within the community.
  • Strong assessment, communication, and critical thinking skills.
  • Supporting hospice care with a multidisciplinary team, including therapies like art and music.
