Job summary

AdvisaCare Hospice is seeking a dedicated Hospice RN to provide compassionate, in-home care in Michigan. You will perform assessments, implement care plans, and collaborate with a multidisciplinary team to ensure comfort and dignity at the end of life.

This full-time role runs 30–40 hours per week with on-call rotation. Requires a current Michigan RN license, at least 1 year hospice experience, empathy, and strong communication with patients, families, and caregivers.

Qualifications

• Initial and ongoing assessment of patient's needs (physical, functional, psychosocial, environmental)
• Implementation of the individualized plan of care and revisions as needed
• Compliance with policies, Medicare and organizational procedures
• Education of patient and family on disease, self-care and end-of-life care
• Supervision of paraprofessionals providing services to patient
• Preparation of clinical and progress notes; participation in IDG conferences
• Participation in Hospice performance improvement program
• Maintain high level of customer satisfaction with clients, families and caregivers
• Communication of information using current process and technology within organization

• Looking for a flexible RN to provide hospice care 30-40 hours a week and include rotation of the on call schedule. This is a great job for someone who needs to work around family schedules but still wants to make a positive impact by utilizing nursing skills. **

For this role, you need to be a registered nurse who sees the value of Hospice care. You will be providing emotional support, with the goal to help patients live their final days with dignity and comfort. Our Hospice RN's will conduct appropriate supervisory visits which allow for clinical observation of performance of assigned team members. Participate and provide feedback to the team for regular interdisciplinary group meetings; on the development, review and revision of the patient/family plan of care.

Requirements

Requirements

• Current Michigan License in good standing
• Minimum 1 year hospice experience
• Empathetic approach to patients and caregivers
• Participate in field visits daily, weekly office meetings as indicated
• Ability to adapt to patient's status and needs on a full time on call basis
