AdvisaCare is seeking a Full-Time Hospice RN dedicated to providing compassionate care in the comfort of home. As a Hospice RN, you'll support patients and families, ensuring dignity during their final days.

The role offers flexibility with 30-40 hours per week and includes an on-call schedule. You'll work collaboratively within a team, conducting visits, and advocating for patient needs.

Qualifications

As a Hospice RN with AdvisaCare Hospice, you'll provide emotional support and compassionate clinical care, helping patients experience their final days with dignity, comfort, and respect.

You’ll also:

• Conduct appropriate supervisory visits
• Provide clinical observation and feedback to assigned team members
• Participate in interdisciplinary group meetings
• Help develop, review, and revise patient/family plans of care
• Advocate for the needs and wishes of patients and their families

Requirements

• Initial and on-going assessment of patient's physical, functional, psychosocial and environmental needs
• Implement the individualized plan of care and revisions as needed
• Compliance with AdvisaCare Hospice Policies and Procedures, Medicare compliance
• Consulting with and educating the patient/family on disease process, self-care techniques, end-of-life care, dealing with issues of ethical concern
• Supervision of paraprofessionals providing services to patient
• Preparing clinical and progress notes, participating in IDG conferences
• Participate in Hospice performance improvement program
• Maintain high level of customer satisfaction with our clients, families and caregivers
• Communication of information using current process and technology within organization

• Current Michigan License in good standing
• Minimum 1 year hospice experience
• Empathetic approach to patients and caregivers
• Participate in field visits daily, weekly office meetings as indicated
• Ability to adapt to patient's status and needs on a full time on call basis
