Job summary

CenterCare Hospice is seeking a compassionate CNA to join our team, providing patient-focused care in the patient’s residence within Pueblo and surrounding areas. The role emphasizes comfort, safety, and dignity, following established care plans and collaborating with the nursing leadership.

Qualifications

• Patient care delivery per plan of care.
• Record vital signs accurately in EMR.
• Assist with activities of daily living and mobility.

Responsibilities

• Demonstrates knowledge and adheres to the plan of care.
• Performs baths, oral hygiene, nail care, foot care, skin care, pressure relief, dressing, feeding and transferring.
• Competently take and record vital signs and notify when out of range.
• Assist patient with activities of daily living and mobility.
• Prepare clinical notes within 24 hours of the visit.
