Job summary

Halcyon Home LLC is seeking a licensed, full-time Speech-Language Pathologist to support geriatric patients in San Marcos, TX and surrounding areas. You will assess, treat, and educate patients and families while collaborating with a multidisciplinary team to achieve optimal outcomes.

The position requires a Master’s in Speech-Language Pathology and state licensure, with home health experience preferred. We offer generous PTO, holidays, and ongoing education opportunities.

Qualifications

• Master's degree in Speech-Language Pathology.
• State licensure as a Speech-Language Pathologist.
• Experience in home health care preferred.
• Proficiency with Homecare Homebase software is a plus.

Responsibilities

• Performs ongoing clinical assessment, including relevant home health evaluations, to identify patient needs.
• Develops and evaluates the plan of care in partnership with the patient and caregiver(s).
• Prepares clinical notes and documentation in a timely manner.
• Provides therapy for speech, language, cognitive, and swallowing disorders per physician order.
• Coordinates with physicians and other health care practitioners involved in the plan of care.
