Join an award-winning team delivering high-quality, orthopedic-focused rehabilitation services directly to patients in their homes and assisted living facilities throughout Central and Southern Fairfield County. We are seeking a Full-Time Physical Therapist (PT) with at least 2 years of clinical experience to help patients recover safely and regain long-term function—all in the comfort of their home environment.
As an In-Home Outpatient (IHOP) Physical Therapist, you will:
Provide one-on-one, outpatient-level physical therapy in home or assisted living settings
Treat post-surgical patients, including total joint replacements, with a focus on safety and independence
Conduct pre-surgical home safety assessments and post-operative follow-up visits
Collaborate with physicians, families, and interdisciplinary teams to create personalized care plans
Educate patients, caregivers, and clinicians on safe mobility and home exercise techniques
Maintain accurate, compliant documentation in line with Medicare Part B home care regulations
Participate in team conferences and contribute to a coordinated care approach

Requirements:
Minimum 2 years of clinical experience as a licensed PT
Bachelor’s, Master’s, or Doctorate in Physical Therapy
Active Physical Therapy license in the state
Must be a U.S. Citizen or Green Card holder (no exceptions)
