Overview

Join an award-winning team delivering high-quality, orthopedic-focused rehabilitation services directly to patients in their homes and assisted living facilities throughout Central and Southern Fairfield County. We are seeking a Full-Time Physical Therapist (PT) with at least 2 years of clinical experience to help patients recover safely and regain long-term function—all in the comfort of their home environment.

As an In-Home Outpatient (IHOP) Physical Therapist, you will:

• Provide one-on-one, outpatient-level physical therapy in home or assisted living settings

• Treat post-surgical patients, including total joint replacements, with a focus on safety and independence

• Conduct pre-surgical home safety assessments and post-operative follow-up visits

• Collaborate with physicians, families, and interdisciplinary teams to create personalized care plans

• Educate patients, caregivers, and clinicians on safe mobility and home exercise techniques

• Maintain accurate, compliant documentation in line with Medicare Part B home care regulations

• Participate in team conferences and contribute to a coordinated care approach

Requirements:

• Minimum 2 years of clinical experience as a licensed PT

• Bachelor’s, Master’s, or Doctorate in Physical Therapy

• Active Physical Therapy license in the state
