Job summary

The ideal candidate will have a current Ohio state license, CPR certification, and preferably prior home care experience. Strong communication, organizational, and time-management skills are essential.

Qualifications

• Current state license to practice as a Physical Therapist in Ohio.
• Current CPR certification.
• Valid driver's license, reliable transportation, and auto insurance.
• Prior home care experience preferred.

Responsibilities

• Complete Start of Care (SOC) evaluations and ongoing therapy visits.
• Develop treatment plans by identifying a problem list and methods to achieve identified goals.
• Coordinate treatment plan with the patient, family, physician, and other team members.
• Adapt treatment to meet the individual needs of the patient.
• Review the quality and appropriateness of the total services delivered.
• Supervise PTAs and HHAs.
• Complete clinical documentation in a timely manner.
