Interim Healthcare Staffing is seeking a licensed Physical Therapist for home care in Minnesota. The role involves evaluating patients, developing and implementing care plans, and coordinating with a care team to ensure high-quality, compliant home health services.

The position requires at least one year of home care PT experience, Medicare/OASIS familiarity, and proficiency with EMR systems. Transportation to patient sites is needed, with a schedule of Monday to Friday, 8am-4:30pm.

Qualifications

• Must have a current license with the Minnesota Board of Physical Therapy.
• Must have a Bachelor's, Master's or Doctorate in Physical Therapy.
• At least one year of PT home care experience.
• Familiar with Medicare visits and OASIS Start of Care process.
• Knowledge of age-related growth and development for primary diagnosis and comorbidities.
• Proficiency with communication and data processing equipment and EMR systems.

Responsabilités

• Assess patients' physical, psychological, social, and environmental needs.
• Develop care plans integrating development and disease processes.
• Maintain records and use them for patient care; proficient with OASIS data set.
• Educate patients and families about self-care and home care.
• Serve as case manager coordinating with RN, SW, home health aide and rehab staff.
• Assist with resolving concerns or complaints and supervise home health aides.
• Refer vulnerable adults to Adult Protection Services when needed.
• Provide skilled clinical care including evaluation, treatment, and plan implementation.
• Anticipate community needs and population characteristics.
