Job summary

The Pines Post Acute in Clintonville, WI is seeking a Full Time Physical Therapist to join our in-house rehab program. Conduct screening, evaluation, and develop care plans to support geriatric patients in a post-acute setting.

New grads are welcome; WI PT license required. You will deliver rehab interventions, discharge summaries, and collaborate with families and caregivers to optimize recovery and quality of life.

Qualifications

• Licensed Physical Therapist with current WI license.
• Open to Physical Therapists at all experience levels; new grads welcome.

Responsibilities

• Screen and evaluate patients with physical functioning disorders and wound care; develop care plans.
• Deliver rehabilitative, skilled, and medically necessary treatment interventions.
• Administer procedures as part of rehab plan including manual techniques, ambulation, exercises, modalities, assistive devices, wound care.
• Provide consultation to patients, families, caregivers, and other service providers.
• Generate discharge summaries ensuring regulatory and clinical compliance.
