Ascension Saint Agnes Hospital Diabetes Care in Baltimore is seeking a skilled Nurse Practitioner to join our endocrine/diabetes team. You will provide comprehensive endocrinology and diabetes care in collaboration with physicians in a highly collaborative, hospital-based practice.

The role requires Maryland NP licensure with prescriptive authority, a Master’s in Nursing, and experience or interest in advanced diabetes management.

Qualifications

• Nurse Practitioner with Prescriptive Authority in Maryland is required.
• Experience in diabetes/endocrinology is preferred.
• MD licensure and CRNP certification are essential.

Responsibilities

• Deliver endocrinology and diabetes care with physician partners.
• Educate patients and families on treatment options and lifestyle changes.

This is an exceptional opportunity for a skilled Nurse Practitioner to join our established, team-based Endocrinology practice at Ascension Saint Agnes. Operating in a highly collaborative environment with strong physician-NP partnerships, this role is perfect for a clinician passionate about advanced chronic disease management. Call structure

• Phone only every 3 weeks
• Access to robust care management, social workers, diabetic educators, collaborative physician team members

Responsibilities

• Deliver comprehensive endocrinology and diabetes care by conducting specialized patient assessments, ordering diagnostic studies, and managing complex chronic health conditions in close collaboration with our physician partners.
• Counsel and educate patients and families regarding individualized treatment options, lifestyle modifications, and community resources to improve long-term disease management and health outcomes.
• Optimize advanced diabetes technologies by interpreting Continuous Glucose Monitor (CGM) and insulin pump data, making precise clinical adjustments, and training patients to confidently use these tools.

Licensure / Certification / Registration

• Nurse Practitioner with Prescriptive Authority credentialed from the Maryland Board of Nursing obtained prior to hire date or job transfer date required.

Experience

• Experience working within a dedicated diabetes/endocrinology practice, or extensive experience managing complex diabetes patients in a primary care setting.
• Proven ability to interpret CGM and pump data, execute clinical adjustments, and lead patient training.
