Position Summary: In compliance with regulation, company policy, and excellent nursing practice, the LPN is responsible for supporting wellness functions that may include, but is not limited to: documentation on electronic health records, communication with residents, families, vendors, physicians, pharmacy and staff; performing and monitoring health metrics for wellness and supporting the care team in addressing resident care issues and concerns.Facilitate Enriching Community Environment:Record the resident’s vital signs e.g. (blood pressure, temperature, weight, etc.)Support ADLs for residents and the resident care teamManage medication orders and documentation in compliance with regulations and follow up as needed with treating physicians and pharmacyDevelop collaborative relationships with third-party service providers like social workers and therapy, to ensure that necessary medical services are received by the residentEducate residents and families in proper healthcare proceduresDocumentation of staff training requirementsDelivery of staff training to support resident careProvide direct assistance to residents with ADLs. ADLs include medication management, dietary management, personal hygiene, and other skills that enable independenceResponsible for ordering and checking medications from the pharmacy to the medication supply areaMay conduct quarterly medication audits for complianceModels strong belief in mission and core valuesActive participation in learning each resident’s interests/preferences to recognize a change in conditionResponsible for ordering, securing, and maintaining stock supplies for the care department as well as maintaining cleanliness and orderExecutes good judgment in the best interest of customers.
