Job summary

Northwest Human Services is seeking a Primary Care Provider (PA/NP) to join our West Salem Clinic in Salem, Oregon. This full-time role supports an outpatient community health center with a team-based approach to care for underserved populations.

The PA/NP will provide coordinated medical services, collaborate with a diverse team, maintain thorough documentation, and contribute to quality improvement initiatives within our integrated care model. Salary ranges based on experience and is DOE.

Qualifications

• Graduate of an accredited medical school, Nurse Practitioner program or Physician Assistant program.
• Licensed to practice in the State of Oregon with prescriptive authority and DEA.
• Must be professionally bondable/insurable.
• Ability and willingness to provide Obstetrical care preferred.
• Team-oriented with strong interpersonal and communication skills.
• Valid Oregon Driver's License and proof of automobile insurance.
• CPR certification in infant, child and adult required.
• Bilingual/bicultural strongly preferred. Bilingual in English/Spanish a plus.
• Previous experience in community health preferred.

Responsibilities

• Provides professional and quality medical services to the community and clinic patients. Consistently meets productivity expectations.
• Consults with supervising and/or on-call and consulting physicians regarding patients with abnormal or unusual medical conditions.
• Maintains appropriate medical records documentation on all patients in accordance with organizational policy. Consistently up to date on charting.
• Meets expectations of above 90% on weekly documentation report and maintains PAQ at less than 75 items at any given period.
• Actively contributes to care team by providing coverage to absent providers, being available to support staff, and serving on improvement teams.
