Job summary

Qualifications

Responsibilities

• Evaluate patients through interviews and gather information for assessments.
• Develop and monitor treatment plans, offering individual, family, and group support.
• Coordinate with community agencies for services needed in patient care.
• Document interactions and ensure confidentiality per CMS and local guidelines.
