Responsibilities

• Evaluates patients through interviews and gathers information; FT minimum 14/day, PT minimum 8/day.
• Provides individual, family and group therapy and plans/ administers treatments.
• Coordinates with community agencies and other professionals for services.
• Maintains patient records, reports and ensures confidentiality.
• Complies with CMS and other regulatory guidelines; supports case discussions.
