We offer Day Shift - Full Time. Join our compassionate team to provide hands-on nursing care in a welcoming medical office or in patients' homes, delivering meaningful care within the Licensed Practical Nurse scope of practice in New York State.

Job Responsibilities
  • Perform direct patient care and procedures within the scope of an LPN in various settings including medical offices, homes, schools, and clinics within the Oneida Indian Nation community.
  • Interview patients about their health history, medications, and symptoms.
  • Measure and record vital signs accurately (height, weight, blood pressure, TPR) demonstrating age-specific competencies.
  • Conduct minor diagnostic procedures such as blood pressure monitoring, lab draws, EKGs, PFTs, Peak Flow, and CLIA-waived tests, reporting results to providers.
  • Assist providers during exams and administer immunizations and medications via IM, sub-q, and intra-dermal routes. Insert and monitor IVs upon certification.
  • Draw blood, prepare specimens, and handle test orders.
  • Maintain infection control standards, clean and prepare instruments, and ensure exam rooms are ready.
  • Manage referrals, schedule and confirm appointments, and coordinate transportation as needed.
  • Maintain supplies, including immunizations, and monitor medical equipment.
  • Document all patient interactions accurately and timely in medical records.
  • Provide patient education on treatment, health promotion, and disease prevention.
  • Facilitate prescriptions and communicate test results to patients.
  • Collect data and prepare reports as directed by supervisors.
  • Participate in committees, staff meetings, training, and continuing education programs.
Qualifications
  • Current NY State license and registration as an LPN.
  • Minimum of three years of diverse clinical nursing experience, including acute care, long-term care, hospice, pediatrics, or community health nursing.
  • Knowledge of PCMH and accreditation processes (preferred).
  • CPR certification.
  • Ability to work indoors and around health risks, with physical ability to lift up to 100 lbs using proper techniques.
