Integrity Care Group seeks Licensed Mental Health Professionals (LSCSW or LCPC) for full-time roles in a nursing home setting in Guthrie, Oklahoma. You will provide mental health counseling, assessments, and interventions within an integrated primary care behavioral health model, serving patients across in-clinic, telehealth, and designated facilities.

Qualifications

• Must have a Master's in an accredited mental health field (e.g., counseling, psychology, marriage and family therapy).
• Active, unrestricted license in state practice (LCSW, LSCSW, LPC, LMFT).
• Minimum 2 years of supervised experience in mental health or health care settings.
• Proficiency with basic PC skills and documentation.

Responsabilités

• Provides mental health counseling, diagnostic evaluation and assessment within an integrated primary care model.
• Performs functional assessments and bio-psychosocial evaluations.
• Plans and implements interventions with patient and caregivers and follows up as appropriate.
• Maintains encounter notes and clinical documentation such as intake assessments and treatment plans.
• Coordinates with referral sources, other providers and team members for coordinated care.
• Coordinates care with contracted facilities and maintains professional relationships.
• Maintains productivity expectations as established at the start of employment.
• Other duties as assigned; this is a safety-sensitive position.
