• Provides mental health counseling, diagnostic evaluation and assessment in various settings (telehealth, in-clinic, designated facilities).
• Completes functional assessments and bio-psychosocial assessments.
• Plans and implements interventions with patient, family, and healthcare providers.
• Maintains encounter notes and clinical documentation (intake, progress notes, treatment plans, discharge).
• Coordinate care with facility personnel and other providers; follow-up on referrals.
