Job summary

Friendship Village is seeking a compassionate Admissions Nurse (RN/LPN) to lead the health center admissions process and maintain open communication with referrals, physicians, and third-party payers. This full-time role runs Monday through Friday, 10:30a–7p, with rotating on-call every sixth weekend.

You will meet new residents and families, handle admission paperwork, and perform initial assessments and care planning to ensure a smooth transition into the community. EOE.

Qualifications

• LPN or RN licensure is required.
• 1+ year of long-term care experience preferred.
• Flexible hours and shifts available.

Responsibilities

• Meet with new residents and families during admission orientation to welcome them.
• Complete admission duties and paperwork for new residents.
• Perform physical assessments, intake forms, medication reconciliation, and initial care plans.

Friendship Village is seeking a compassionate and energetic individualto join our Health Center team as an Admissions Nurse that is responsible for leading and driving the Health Center admissions process and for fostering and maintaining open, effective communication between the facility and referral sources, physicians, and third-party payers.This is a full-time, Monday-Friday 10:30a-7p position with rotating on call every 6th weekend. Here’s a snapshot of what you’ll be doing each day as an Admissions Nurse (RN/LPN):

• Meet with new residents and family members upon arrival to community for general admission orientation ensuring a friendly welcome to the community.
• Complete admission duties and paperwork where the new resident is assigned.
• Complete physical assessment, in-take forms, medication reconciliation from the discharge summary, initial care plans, and other forms relevant to the admission process.
